Leukemia and lymphoma are frequently associated with reduced IFN-gamma production by activated T cells. Among patients with chronic myelogenous leukemia (CML), this appears to be related to defects in the maturation and function of antigen-specific cytotoxic T cells. Here, we have examined the direct effects of IFN-alpha on allogeneic MLR in patients with CML, the extent of defects in IFN-gamma production by leukemic blasts, and the effects of IFN-alpha on IFN-gamma production and cytotoxicity by CML cells. Peripheral blood mononuclear cells from patients with CML were found to be less responsive to stimulation with PHA or alloantigens. Although the frequency of Th1 cells, as measured by IFN-gamma production, was reduced in patients with CML, IFN-alpha was unable to restore IFN-gamma production to normal levels. There was, however, a consistent synergistic effect of IFN-alpha and IL-12 on IFN-gamma production by CML cells. Although IL-12, in combination with rIL-18, could expand CTL precursors from patients with CML, IFN-alpha could not augment IL-12-induced lymphokine production by CML blasts. The number of CTL precursors present in the peripheral blood of patients with CML was found to be reduced, and IFN-alpha could substantially increase the number of CTL
